Bone Marrow Transplant Cost in India (2026): Full Breakdown for International Families

For most treatments, travelling abroad is about saving money. For a bone marrow transplant, it is often about whether the treatment happens at all.

A transplant in the United States can cost 300,000 dollars or more. Families in Nigeria, Iraq, Bangladesh, Kenya or Uzbekistan facing that number for a child with thalassemia are not comparing options. They are looking for the one place the sum works.

In India, the same procedure costs between roughly 12,000 and 50,000 US dollars depending on the type. This guide explains where in that range your case is likely to sit, and what else you will spend before you fly home.

Bone marrow transplant cost in India: 2026 price ranges

The word transplant covers several very different procedures. The cost gap between them is large, and it is driven mainly by the donor.

Transplant type

Cost (INR)

Cost (USD)

Autologous (your own stem cells)

₹10,00,000 – ₹18,00,000

11,500 – 20,700

Allogeneic, matched sibling donor

₹18,00,000 – ₹28,00,000

20,700 – 32,200

Haploidentical (half-matched family donor)

₹25,00,000 – ₹38,00,000

28,700 – 43,700

Matched unrelated donor (MUD)

₹30,00,000 – ₹45,00,000

34,500 – 51,700

Cord blood transplant

₹28,00,000 – ₹42,00,000

32,200 – 48,300

Autologous transplants use your own stem cells, harvested before high-dose chemotherapy and returned afterwards. No donor, no rejection risk, no immunosuppression. Common for myeloma and some lymphomas. The cheapest and the most predictable.

Allogeneic with a matched sibling is the standard for thalassemia, aplastic anaemia and many leukaemias when a brother or sister matches. Roughly a one in four chance among full siblings.

Haploidentical uses a parent or a half-matched sibling. It has transformed access in South Asia and Africa, because almost every patient has a half-matched relative. It costs more because of the extra processing and the heavier immunosuppression that follows.

Matched unrelated donor transplants pull cells from an international registry. The registry search and the courier transport of donor cells account for most of the additional cost, often 12,000 to 18,000 dollars on their own.

What the package covers

A BMT package at an Indian transplant centre generally includes the pre-transplant workup, HLA typing for patient and family donors, the conditioning chemotherapy, stem cell harvest and processing, the transplant itself, the isolation room stay of three to four weeks, supportive medications, blood and platelet transfusions during admission, transplant physician and team fees, and follow-up until discharge.

That admission is the expensive part. A HEPA-filtered isolation room with round-the-clock specialist nursing is not a normal hospital bed, and the daily cost reflects that.

What sits outside it

  • Complications. Graft-versus-host disease, serious infection or graft failure extend the stay and add cost. This is the single biggest variable.

  • Extended isolation beyond the packaged number of days.

  • Donor expenses. Travel, visa, accommodation and workup for a donor flying with you.

  • Registry fees for unrelated donors, and the courier cost of moving cells internationally.

  • Post-discharge care to day 100, including outpatient visits, chimerism testing and repeat blood work.

  • Immunosuppressive and antiviral medication for months after discharge.

  • Accommodation and food for the family during the outpatient phase.

Budget 20 to 30 percent above the package figure. Transplant is the one procedure where quoting the package alone misleads families, and honest coordinators say so upfront.

What drives your number up or down

Donor type. As the table shows, this is the largest factor by a wide margin.

Diagnosis. Thalassemia in a young child with a matched sibling is comparatively straightforward. Relapsed acute leukaemia in an adult is not.

Age and general condition. Children under ten usually recover faster and use fewer resources than adults over fifty.

Complications. GVHD is common after allogeneic transplant and ranges from mild skin involvement to something requiring weeks of intensive management.

Hospital. A flagship transplant unit in Delhi or Mumbai costs more than an equally accredited centre in a smaller city.

Length of isolation. Priced per day once past the packaged days.

How India compares

Country

Approximate allogeneic BMT cost (USD)

India

20,000 – 50,000

Turkey

45,000 – 70,000

Thailand

60,000 – 90,000

UAE

90,000 – 150,000

Singapore

120,000 – 180,000

UK

150,000 – 250,000

USA

250,000 – 450,000

The saving is 70 to 85 percent, and unlike smaller procedures it holds comfortably even after months of accommodation.

The donor question

Before anything else happens, you need to know whether a donor exists.

The first step is HLA typing. Blood samples from the patient and from siblings and parents are tested for tissue compatibility. A full sibling has about a 25 percent chance of matching. Where no sibling matches, a half-matched parent or child usually can, which is why haploidentical transplants have become so widely used.

Many Indian centres will run HLA typing on samples couriered from abroad, so you can answer the donor question before booking a single flight. That is worth doing. It changes both the cost bracket and the plan.

If your donor is travelling with you, they need their own visa. A donor is not automatically covered under the medical attendant category, so raise it early with the hospital and with whoever is preparing your visa file. Applications are made through indianvisaonline.gov.in. Our Indian medical visa guide explains the categories.

How long you actually stay in India

This is where families most often underestimate.

  • Weeks 1 to 2. Arrival, workup, donor evaluation, central line placement.

  • Days minus 7 to minus 1. Conditioning chemotherapy, sometimes with radiation.

  • Day 0. The stem cell infusion itself, which takes a few hours and looks anticlimactic.

  • Days 1 to 21. Isolation while counts recover. The hardest stretch, medically and emotionally.

  • Discharge, around week 4 to 6. Only when engraftment is confirmed and counts are stable.

  • To day 100. Outpatient monitoring, living close to the hospital, with frequent visits.

For an autologous transplant, plan on six to eight weeks in India. For an allogeneic transplant, plan on three to four months, and be ready for longer if GVHD appears.

Nobody wants to hear four months. Booking as if it will be six weeks is worse.

Where transplants are done

India's established BMT programmes sit at Apollo, Fortis Memorial Research Institute in Gurugram, BLK-Max in Delhi, Amrita in Kochi, Tata Memorial in Mumbai, and several Narayana and Manipal units.

Check that the unit holds current NABH or JCI accreditation, then ask each one the same question: how many transplants of my exact type does this unit do per year, and what is your day-100 survival for that group? Any serious transplant physician answers without hesitation. Volume matters here more than in almost any other procedure.

More on selecting a unit in our hospital selection guide, and procedure detail on our bone marrow transplant in India page.

Budgeting the whole journey

For an allogeneic transplant with one attendant and a family donor, a realistic total looks like this.

  • Transplant package: 20,000 to 38,000 dollars

  • Complication contingency: 5,000 to 10,000 dollars

  • Medical visas for three people

  • Return flights for three

  • Accommodation for three to four months near the hospital, roughly 900 to 2,000 dollars a month

  • Food, local transport and interpreter support

  • Post-transplant medication for six to twelve months

Most families land somewhere between 35,000 and 55,000 dollars all in. That still sits well below the surgical package alone in the Gulf, Singapore or the UK.

Practical ways to keep the cost down

Complete HLA typing before you travel, so the transplant type and price are known in advance. Prefer a family donor over a registry search where one is available, since the difference is often more than 15,000 dollars. Ask for a written cap on packaged isolation days and a per-day rate beyond it. Compare two or three transplant units rather than one. Look for serviced apartments rather than hotels for the long outpatient phase, since three months in a hotel costs more than the difference on the surgical package.

What not to cut: unit experience, or the day-100 follow-up. Leaving early is the most expensive decision in the entire process.

What MediVoyage does

We arrange HLA typing coordination, send your file to transplant units that do high volumes of your specific diagnosis, and bring back written opinions with itemised estimates including expected complication costs rather than the headline package alone.

We also handle visa invitation letters for the patient, attendants and the donor, airport pickup, long-stay accommodation near the transplant unit, interpreters, and follow-up reports to your haematologist at home through the day-100 period and beyond. None of it is charged to you.

Send us your reports and family HLA details and we will come back within two working days.

Frequently asked questions

How much does a bone marrow transplant cost in India? Between roughly 11,500 and 52,000 US dollars in 2026, depending on transplant type. Autologous sits at the bottom of that range, matched unrelated donor transplants at the top. Most family-donor allogeneic transplants fall between 20,000 and 38,000 dollars.

What is the success rate of BMT in India? For thalassemia in children with a matched sibling donor, leading Indian units report success rates around 85 to 90 percent. Results vary widely by diagnosis, age and donor type, so ask your chosen unit for figures specific to your case rather than a general number.

How long will we need to stay in India? Six to eight weeks for an autologous transplant. Three to four months for an allogeneic transplant, covering the day-100 monitoring window. Longer if complications develop.

Does my donor need a separate visa? Yes. A donor is not automatically covered by the medical attendant category. Flag the donor early so the hospital can issue the right invitation letter.

Is a haploidentical transplant as good as a matched one? Outcomes have improved substantially and haploidentical transplants are now standard practice at experienced centres. A fully matched sibling remains the first preference where one exists, but a half-matched parent is a genuine option rather than a fallback.

What is graft-versus-host disease? A reaction where donor immune cells attack the recipient's tissues, most often skin, gut or liver. It is common after allogeneic transplant, ranges from mild to severe, and is the main reason costs and stay length vary so widely.

Can a bone marrow transplant cure thalassemia? It is currently the only established curative treatment for thalassemia major. Results are best in younger children with a matched donor and limited iron overload, which is why haematologists push for early referral.

Is treatment covered by insurance? Some international policies cover transplants performed abroad, many do not. Get written confirmation from your insurer before travelling, including whether they reimburse against Indian hospital invoices.

Where to start

Answer the donor question first. Everything else, including the price, follows from it.

Get HLA typing done for the patient and immediate family, send the reports and typing results to two or three experienced Indian transplant units, and ask each for an itemised estimate that includes expected complication costs rather than the package alone.

Bone marrow transplant cost in India is the reason many families can proceed. The reason to choose carefully is that in transplant, more than anywhere else, the experience of the unit shows up in the outcome.

Send us the file when you are ready. We will handle the rest of the coordination.
